Essential Details to Include:
Family Information: The caregiver’s full name, address, and date of birth, along with the first and last names and birthdates of all children in their care. Include answers to the two background questions.
Validator Information: Your full contact information and organization affiliation
Details about the kinship care situation you're validating
Information about the family's support needs
